What to Bring

Please bring the following items to your appointment:

• A method of payment – fees apply for all exams. For any questions regarding our fees, please refer to our Pricing Policy.
• Your Ontario Health Card and any applicable vision insurance information.
• Your current pair of prescription glasses.
• Your contact lenses along with the box containing the prescription and brand information.
• A list of any current medications.
• A pair of sunglasses, as your eyes may be sensitive to light after dilation.

A Few Reminders:

• We aim to stay on schedule, and kindly ask that you arrive 10 minutes before your appointment.
• We require a 24-hour notice for any appointment cancellations. Failure to do so will result in a $50 cancellation fee. This helps us accommodate other patients and maintain our schedule.
• Parking is available at the front of the building and along the side.
